At Dorset County Hospital NHS Foundation Trust we deliver outpatient appointments in different ways. Please familiarise yourself with the information relevant to your appointment type. Video consultation appointments (Attend Anywhere)
Attend Anywhere is a secure web-based video platform that allows you to have a consultation from your home or own private space. More information is available here and below in the leaflets and questionnaires section.

Outpatient leaflets and questionnaires
To find the leaflet or questionnaire relevant to your outpatient appointment, please select the referenced leaflet or questionnaire from the list below. Questionnaires (or similar) must be completed before you attend and you will be required to print this out, complete it, and bring it with you.
