Transition Service

The Transition Service is being developed to support young people with long term health conditions, and their parents/carers, to prepare, guide and empower them with a structured transition process from paediatric to adult services. This aims to improve young people’s health outcomes, quality of care and patient experience. The Transition team works with health professionals within the Trust and those in Primary Care, Social Care, NHS Dorset, Dorset Council and Voluntary Organisations to develop a robust network of care for our young people and their parents/carers.
